Information Available in this Collection
The Kentucky Death Index Records cover a large number of records between 1911 and 1999 and contain over 5.85 million births. The records typically include the following information:
- Child’s Name
- Date of Birth
- Gender
- Mother’s Name

- Volume Year
- Volume Number
- Certificate Number
- Record Code
Please note the original data does not always provide full information and there are fewer records in the earlier years.

The Kentucky Vital Statistics office has on file birth certificates from 1911 to the present.
It does not have records of births occurring prior to 1911, however except delayed records of births for persons born before 1911 which have been established by affidavits and documentary evidence.
Certified birth certificates can be ordered and the required fee paid by phone, mail, Internet or by visiting the office.
Information needed to obtain a certificate:
- Full name at birth of the person for whom the certificate is requested
- The month, day and year of birth
- The county of birth
- The mother's maiden name
- The father's name
